Registration Information

We are excited that you want to join our ELC family!  After your application has been accepted and approved, please follow these steps to complete your registration.

1. You will receive a packet of information from the Early Learning Center office.  Please fill out all of the forms inside this packet and return them to the office prior to your child's first day.  Here is a list of the paperwork that will need to be filled out inside of this packet.
2. Obtain a copy of your immunization record from your clinic or the Health Department. If your child is behind in immunizations, you will need to make an appointment to visit the Health Department and you can obtain a waiver there.   
3. You will also need to make an appointment with a local health professional to have a brief physical and fill out this Health Appraisal. You do have 30 days to this after enrollment. If you have had a recent appointment, your clinic will likely fill out the form for you based on that appointment, rather than making you schedule another.
4. Send an email (secretary@copperluth.org) with your desired schedule and we will email you back a draft contract once it has been approved.
5. If any changes are needed, please connect with the Director of the ELC to finalize your schedule and contract.
